In full: "When I gave this photo to Jack Lord to autograph on the set of Hawaii 5-0, he asked 'where did you get this?' I told him my father got it for me from their PR Department. His reply was, 'they told me they didn't have any. Who's your father?' He then signed it for me, though there was a pre-signed signature on the back." Accompanied by inscribed signature: "For Susan/Aloha/Jack Lord", 5½x7¾. Ink note (unknown hand) at upper margin: "August 6, 1969/Filming on Magic/Island -/Make-up Man - Kenister Keester Sweeney".
